Talk Summary
In this lecture, Dr. Bonafide will review the broad range of observational methods his team has used to understand the practice of clinicians working in the hospital, their interactions with technology, and the downstream implications for patients. He will describe using video recording to understand alarm fatigue and measure alarm response time, merging complex datasets of telephone calls and text messages with medication administration data to measure the impacts of interruptions on errors, and engaging teams of hospitalists in 56 centers to collect data on pulse oximetry overuse to inform de-implementation efforts.
About the Speaker
Dr. Bonafide is a pediatric hospitalist and Research Director for Pediatric Hospital Medicine at Children’s Hospital of Philadelphia (CHOP) and Assistant Professor of Pediatrics at the University of Pennsylvania. His research focuses on discovering the best ways to identify deteriorating patients, and the best mechanisms for rapidly responding to those patients. Early in his career, he realized that misuse and overuse of physiologic monitoring contributed to alarm fatigue, which in turn made it more challenging to identify deteriorating patients. With funding from an NHLBI Career Development “K” Award, Dr. Bonafide used innovative video methods to measure alarm fatigue in nurses during actual patient encounters in the hospital. He is currently running the first phase of the EMO study, a 56-hospital project funded by an NHLBI U01 aiming to measure continuous pulse oximetry overuse in bronchiolitis and develop strategies to de-implement the practice in stable children. Dr. Bonafide also directs CHOP’s Patient Safety Learning Laboratory sponsored by AHRQ focused on re-engineering the ways we monitor patients in the hospital as well as in the home care setting. In addition, Dr. Bonafide co-founded CHOP’s Pediatric Hospital Medicine Fellowship and co-directs a pediatric hospital medicine research fellowship at The Children’s Hospital of Philadelphia funded by an NICHD T32, the Pediatric Hospital Epidemiology and Outcomes Training (PHEOT) Program. In addition to his publications in journals such as JAMA, JAMA Pediatrics, Journal of the American Medical Informatics Association, and Pediatrics, his work has been featured on National Public Radio and in The Wall Street Journal and The Washington Post. Dr. Bonafide serves as an Associate Editor of the Journal of Hospital Medicine and is an Editorial Board Member of Pediatric Quality and Safety.
