The purpose of the study is to learn about how to improve access to care during young adulthood and to assess if telehealth is an acceptable form of care.

Eligibility

1.) Young adults ages 16 to 25 years of age on their date of recruitment who are living with Type 1 diabetes (at least 6 months)
2.) Currently receiving care at Children's Hospital Los Angeles (CHLA)
3.) Have California Children's Services (CCS) and/or qualifying private insurance (excluding United and private Healthnet insurance)
4.) Does not plan to transfer out of CHLA within the next year
