CME Application

Presentation Setting

Our Pediatric Grand Rounds presentations occur every Friday morning at 8 a.m. 

Clinicians gather in the John Stauffer Conference Room in the Marion and John E. Anderson Pavilion which can comfortably seat approximately 275 individuals. 

Presentations are projected onto two screens at the front and rear of the room so presenters can view their slides without needing to break rapport with their audience by turning to information residing on the wall behind them. 

Forms to Complete

In order for our hospital to provide CME and CEU credit hours to attendees, we ask that you work with our Academic Affairs office to provide completed packets 4 weeks ahead of your scheduled presentation.  Forms to download are provided for you below:

  Please complete the entire packet and sign off on all forms and submit to Martha Bustamante using the contact information below.
   
• CME Appendix
  This document provides additional information for your reference as you prepare to present to our faculty and staff.

Once you have completed and submitted your presenter packets, our staff will complete their own Organizer Packet to request authorization to provide CME and CEU credits.  For your reference, the latest version of this document is provided below.
