Prior to receiving an outpatient speech and language evaluation, your child will need the following:
1. A hearing test
This may be completed here or a hearing test from another clinic/facility or one from your child’s school may be faxed pending approval from our front office (link to phone number).
2. Authorization from your insurance company
This may require a pediatrician/primary care physician visit; call our front office (link to phone number) to schedule an appointment 3 days after you fax the authorization to our office.
3. History form
A history form will be mailed or given to you prior to the evaluation. Please fill this out as completely as possible and bring it with you to the appointment.
4. Relevant educational and therapeutic information
If your child has an IEP in place at school or has had a prior speech or developmental evaluation, please bring a copy to the evaluation.
