 What to Bring to an Endocrinology and Metabolism Appointment
For your first appointment, please bring the following items:
- Recent lab work (any work that was not faxed originally with the patient's referral to our clinic)
- Insurance card, co-payment, authorization (if required)
- Blood Glucose Monitor and Blood Glucose Logs (for children with diabetes)
- Actual X-Ray film (only if one was done) (only for Growth issues)
- Legal Guaardianship documentation (if you are not the natural parent)
