Basic Employee Life InsuranceChildren's Hospital Los Angeles provides you with a basic life insurance benefit at no cost to you.
Supplemental Employee Life InsuranceIf you want to buy additional life insurance with after-tax dollars, you may elect supplemental insurance in the following amounts:
Voluntary Spouse Life InsuranceYou may purchase life insurance for your spouse. The premium for dependent life insurance is deducted from your paycheck after-tax. You may purchase $5,000, $10,000, $25,000, or $50,000 of life insurance coverage for your spouse, or up to 50% of your coverage amount, whichever is less. You must be enrolled in the Supplemental Employee Life insurance in order to cover your spouse or your registered domestic partner. Voluntary Child Life InsuranceYou may purchase life insurance for your child(ren). The premium for dependent life insurance is deducted from your paycheck after-tax. You may purchase 5,000 of life insurance per child for children from birth to age 19 (or to age 25 if a full-time student). You must be enrolled in Supplement Employee Life insurance in order to cover your child(ren). Basic Accidental Death and Dismemberment (AD&D)The accidental death and dismemberment (AD&D) plan is designed to pay benefits in the event of an accidental death or serious injury. Basic AD&D is offered at no cost to you.
Supplemental Accidental Death & Dismemberment (AD&D)You also have the availability to purchase Supplemental AD&D. This is a separate enrollment from your basic life insurance coverage. You may elect Supplemental AD&D insurance in the following amounts:
